Online application for severely injured families 

1. A copy of deployment orders which show that the child’s parent is National Guard or Reserve deployed overseas and has at least 60 days remaining on the orders. For Air Components, there must be at least 30 days remaining on CED orders.

2. One of three forms of identification – either A) a copy of the completed Form 1172, Application for Uniformed Services Identification (DEERS Form), OR B) a copy of the child’s birth certificate if he/she is the biological child of the deployed service member, OR C) copy of DEERS/MilConnect Service Member profile page with child listed as dependent;

3. Documentation of the cost of the activity, such as a flyer, brochure, or a letter from the organization which clearly states the cost of the program. Letters must be typed (hand written fees will not be accepted) and on official letterhead. Grant requests for tutoring services provided by an individual must include a copy of the tutor’s teaching certificate.

4. Grant approvals are always subject to the availability of funds.

1. If the service member is active duty, submit a copy of the most recent orders to a WTU or medical hold.

2. If the service member is a medically retired veteran, submit a copy of the VA paperwork that contains the disability percentage and diagnosis and form DD214.

5. A letter from the service member/veteran’s case manager, certifying that the service member/veteran is A) currently receiving treatment for injuries, B) sustained combat-related injuries while deployed in support of any post 9/11 overseas contingency operation.

6. Grant approvals are always subject to the availability of funds.

  • Only one check will be issued per child.
  • No single grant can exceed $500.
  • Grant checks are issued directly to the organization providing the activity or tutoring – there are no exceptions.
  • Applications for grants will be acknowledged in writing within 10 business days of receipt. The child will receive a congratulatory packet notifying him/her of the award. The check is enclosed in the child’s packet for the family to hand deliver to the service provider.
  • All funding from the grant must be used for the purpose stated on the application or full repayment of the grant will be requested.
  • Incomplete and illegible applications will not be processed.
  • Applications may be deferred if further information is needed to verify eligibility. Grants are awarded at the sole discretion of the grant committee of Our Military Kids, Inc.
