The activity is eligible for a grant if it falls in one of those four categories. We cannot award grants for child care, school tuition, or religious mission trips.

1. A copy of deployment orders which show that the child’s parent is National Guard or Reserve deployed overseas and has at least 60 days remaining on the orders;
2. One of three forms of identification – either A) a copy of the completed Form 1172, Application for Uniformed Services Identification (DEERS Form), OR B) a copy of the child’s birth certificate if he/she is the biological child of the deployed service member, OR C) copy of DEERS/MilConnect Service Member profile page with child listed as dependent;
3. Documentation of the cost of the activity, such as a flyer, brochure, or a letter from the organization which clearly states the cost of the program. Letters must be typed (hand written fees will not be accepted) and on official letterhead. Grant requests for tutoring services provided by an individual must include a copy of the tutor’s teaching certificate.
4. Grant approvals are always subject to the availability of funds.
1. If the service member is active duty, submit a copy of the most recent orders to a WTU or medical hold.
2. If the service member is a medically retired veteran, submit a copy of the VA paperwork that contains the disability percentage (rated 30% or more) and diagnosis and form DD214.

A letter from the service member/veteran’s case manager, certifying that the service member/veteran is A) currently receiving treatment for injuries sustained in support of any post 9/11 overseas contingency operation, B) sustained combat-related injuries while deployed in support of any post 9/11 overseas contingency operation.
6. Grant approvals are always subject to the availability of funds.
